The 4 Steps of Effective Failure Analysis

Use CMMS records, maintenance logs, and operator notes.
2. Conduct a Visual Inspection & Perform Testing
Now zoom in. Is the component cracked, corroded, misaligned, or sheared?
Use your five senses and tools like thermal imaging, ultrasonic testing, or vibration analysis.
3. Identify the Failure Mode
Was this a fatigue failure, a brittle fracture, fretting, thermal degradation, or contamination?
Match the failure with the forces and stresses it faced.
4. Implement CAPA (Corrective and Preventive Action)
Don’t stop at “replaced the part.”
Investigate root cause. Was the wrong fastener used? Could a Nord-Lock washer prevent vibration loosening?
Document lessons learned and finally, train your team.
